A 4-year-old boy presented with fatigue and frequent falls. After falling, he could not stand up using his legs and would instead push himself up with his hands. The aminotransferase levels were elevated. A diagnosis of Duchenne muscular dystrophy (DMD) was made. Glucocorticoid therapy with deflazacort was initiated at the time of diagnosis to slow the progression of motor and cardiopulmonary dysfunction and mitigate the development of scoliosis. The videos show testing of timed rise from the floor before and after glucocorticoid therapy. Read the full case details and learn more about DMD in “A 4-Year-Old Boy with Fatigue, Imbalance, and Frequent Falls,” a Case Record of the Massachusetts General Hospital: nej.md/4h7h0SP

A 35-year-old woman presented with a 20-day history of a painless nipple ulcer without other associated symptoms or skin findings. Histopathologic analysis of a nipple-biopsy specimen showed epidermal necrosis with inflammatory exudate, dermal capillary proliferation, and dense perivascular lymphoplasmacytic infiltrates. Immunohistochemical analysis of the biopsy is shown. What is the most likely diagnosis? Submit your answer and see how others responded: nej.md/IC10012026
