The #1 gripe for me: locally, when you use the worktree option, you get this error when Claude creates things outside of the worktree and then links them in chat. Claude likes to do this a lot, all on its own, so this is a common struggle. Illustrated with the screenshot..

To add to this, the most common scenario that this happens in is with subagent spawns. If your subagents are using separate worktrees (they are), and you use custom worktree paths (worktrees are created outside of the 'project' path), then you'll run into this constantly.
